Your Life Your Choice Program

Your Life Your Choice is an individual funding program provided by the Queensland Government to support people with disability to purchase products and services.  Participants may choose the level of responsibility they wish to take to manage these funds through our range of service levels outlined below:

Service Level 1 – Participant takes full responsibility: 4% of your total funding package
Service Level 2.1 – Participant takes most responsibility:  $250 per plan + 4% of your total funding package for planning and funds facilitation
Service Level 2.2 – Participant takes most responsibility: $250 per plan + 6% of your total funding package for planning and funds management

To assist in your decision to choose the right host provider, we offer to provide you with a unique FREE Initial Consultation and Personalised Support Proposal.  Complete our online questionnaire and we will send you a comprehensive, personalised support proposal based on the information you provide.

For eligible participants, we also have a range of member's services for people with brain injury or developmental disability including:

• Taking Charge Fundraising Program which helps to meet disability needs not met through other funding programs.
• Holiday/Respite Unit at Caloundra - only members may access the unit during school holiday periods, however, non holiday periods are available for Your Life Your Choice participants.

Members are not required to be Australian citizens.

Your Life Your Choice Services and Fees

Service Level 1 – You take full responsibility: 4% of your total funding package

The person accepts full responsibility.  Developing Foundation will assist you with:

Fund Facilitation

• Provide you with information on how the service works
• Transfer agreed advance payments to your nominated bank account
• Hold your funding and monitor the level of your funds
• Provide you with financial templates to record your purchases
• Provide you with an up-to-date quarterly report on the balance of your package
• Submit quarterly reporting and acquit funds

Plan Review & Endorsement

• Review your plan to ensure your purchases are consistent with your Department of Communities approved services categories
• Endorse your plan for support and funding

Service Level 2 – You take most responsibility:  $250 plus 4% or 6% of your total funding package

Service Level 2.1: $250.00 + 4% of your total funding package (funds facilitation)

The person accepts most of the responsibilities. Developing Foundation will assist you with:

Plan Development

• Develop a plan that will determine aims, outcomes, responsibilities and time frames
• Provide you with a list of services and supports available that caters for each individual needs
• Work with you to identify your choice of preferred supplier
• Endorse your plan for support and funding
• Initial review of YLYC Plan one month after implementation
• Monitor and review YLYC Plan on a six monthly basis.
• Collate your support worker requirements
• Broker support worker services or provide information on management of support workers

Fund Facilitation

• Provide you with information on how the service works

Plan Development - $250.00 per plan

• Develop a plan that will determine aims, outcomes, responsibilities and time frames
• Provide you with a list of services and supports available that caters for each individual need
• Work with you to identify your choice of preferred supplier
• Endorse your plan for support and funding
• Initial review of YLYC Plan one month after implementation
• Monitor and review YLYC Plan on a six monthly basis.
• Collate your support worker requirements
• Broker support worker services or provide information on management of support workers

Fund Management – 6% of your total funding package

• Provide you with information on how the service works
• Work with you to prepare a budget
• Make payments on your behalf
• Hold your funding and reimburse your invoices for the services and/or supports you receive
• Monitor your spending and manage your records
• Provide you with an up-to-date quarterly report on the balance of your package
• Submit quarterly reporting and acquit funds
