Riverwalk is a sponsored 5km walk being held in various locations to raise funds for children and adults with brain injury and developmental disability.

Member Application Form - individuals with a brain injury or developmental disability or a carer

A person with brain injury or developmental disability 18 years of over or a carer of an adult of child with brain injury or developmental disability are eligible to become members of The Developing Foundation Inc.  To join, please download and complete the attached form and return to info@developingfoundation.org.au, fax to 07 3870 1123, or mail to 14 Munro Street, Auchenflower, QLD 4066

Member Application Form - volunteers assisting the organisation or individual families

Complete this form if you are assisting a family to fundraise or volunteering directly with The Developing Foundation.  The $25 annual membership fee is paid by the organisation on behalf of volunteers.

Membership Renewal / Cancellation Form - for all members

Please complete the form when you wish to renew or cancel your membership.

Online Fundraising Registration

Members of The Developing Foundation may register to raise funds online for a recipient with brain injury or developmental disability.  Please complete the form if you wish to put up a personal fundraising page on our website.  Note that fundraisers must complete fundraising training prior to commencement of fundraising.  Training is available online, over Skype or in our Auchenflower office.

Riverwalk Fundraising Registration

Members of The Developing Foundation may register to raise funds at our Annual Riverwalk for a recipient with brain injury or developmental disability.  Complete the form at this link.  Please note that fundraisers must complete fundraising training prior to commencement of fundraising.  Training is available online, over Skype or in our Auchenflower office.

Non members may also register but will need to become a member of the organisation by completing the Member Application Form above.

External Event Registration Form

Members of The Developing Foundation may register to raise funds through an event for a recipient with brain injury or developmental disability.  Complete the form at this link.  Please note that fundraisers must complete fundraising training prior to commencement of fundraising.  Training is available online, over Skype at our Auchenflower office.

G11 Form For events with over 100 attendees

This is a form required by our insurer and should be submitted when registering an external fundraising event with more than 100 people attending.

Claim Form - Reimbursement to Member

Members complete this form when claiming expenses from their fundraising account. Complete this form when the funds are to be paid to the Member.  Note:  payments are not paid direct to families unless for minor expenditures from suppliers who require immediate payment or as otherwise arranged with the office. Refer to Self Directed Funds policy for allowable expenditure. If your account does not have the required funds to pay accounts, invoices will be returned to you for direct payment.

Claim Form - Reimbursement to Service Provider

Members or registered Service Providers may complete this form when claiming expenses. Complete this form when the funds are to be paid to the Service Provider.  If a member account does not have sufficient funds, invoices will be returned to member for direct payment.

Register or Cancel a Service Provider

Please complete this form to register or cancel a Service Provider.  By registering a Service Provider you are agreeing that they may submit a claim form for services provided to you. It is your responsibility to cancel a registration. If there are insufficient funds, accounts will be forwarded to members for direct payment.

Authorisation of Regular Payment to Nominated Service Provider

Members complete this form to register a Service Provider and/or to set up regular payments.  Registered Service Providers may complete and submit claim forms directly to The Developing Foundation.  It is the responsibility of the member to cancel Service Provider registration or regular payments.  If there are insufficient funds, accounts will be sent direct to members for payment.

Claim Form and Authorisation of Regular Payment to Nominated Service Provider - Download Word Document

Members complete this form to set up regular payments to a service provider or to set up regular payments to a nominated service provider.  These payments continue to end date or until no funds are available in member's account or until advised by member.
